- Battle of Hong Kong (8 December 1941 ~ April 1942)
- The Solomon Islands campaigns (16 September 1942 ~ 13 November 1942)
- repair & refit at the Mitsubishi Yokohama (14 December 1942) - a Type 21 air-search radar was installed. The No. 7 14 cm gun was replaced by an unshielded twin 12.7 cm/40 Type 89 gun. The No. 5 gun was removed. 2 x triple mount 25-mm guns were added (Total 10 x 25-mm AA guns) & 1 x quadruple Type 93 13.2 mm machine gun was added in front of bridge.
- converting to an anti-aircraft cruiser at Mitsubishi Yokohama (17 January 1944 ~ 14 September 1944) - All of her remaining 14 cm guns were removed & 2 x twin 12.7 cm/40 Type 89 guns were installed. Type 96 25 mm AA guns was increased to 50 barrels. The catapult and seaplane equipment were removed. Type 13 air-search, Type 21 air-search and Type 22 surface-search radars were fitted or modified. Sonar and depth charge rails were added.
- Battle of Leyte Gulf (20 October 1944) - Isuzu rescued 480 survivors from the sunken aircraft carrier Chitose. Isuzu was damaged while rescuing survivors from the sunken aircraft carrier Chiyoda (千代田).
- Isuzu was sunk by USN submarines off Sumbawa, Java Sea 7 April 1945 (on same date as battleship Yamato was sunk)
